Trigger Point Dry Needling

Trigger point dry needling is a physical therapy treatment technique used to address painful limitations caused by both active and latent trigger points. Trigger point dry needling in conjunction with manual physical therapy, therapeutic exercise and other skilled physical therapy interventions has shown to be successful in accelerating treatment of numerous musculoskeletal pathologies.

Trigger point dry needling requires highly specialized training. Few medical professionals are trained in the evaluation and treatment of trigger points. Trigger point dry needling can be very effective in treating pain of a myofascial (chronic pain) origin. Dry needling has been the difference maker in many patients’ conditions and is an option to consider for the relief of pain.

How does trigger point dry needling work?

Trigger Point Dry Needling is thought to work by disrupting a reflex arc of contracted tissue to allow improved blood flow and healing to the area, as well as stimulating biochemical changes. Dry needling utilizes a very thin filament needle. The needle is inserted into the trigger point and a local twitch response is ideally elicited. This twitch response is an involuntary contraction of the muscle that allows the muscle to relax. The needle used is similar to an acupuncture needle, but is not considered acupuncture and is based on Western physiologic approaches. Electrical stimulation with heat can also be applied to the needle to elicit these twitches to help the muscle relax.

What are trigger points?

Trigger points are extremely irritated contraction knots found in tight bands of muscles. They are a common cause of pain and muscle tightness and can affect the local area of pain. Trigger points can refer to parts of the body, which can all effect strength, flexibility, and coordination of muscles.

What does Dry Needling feel like?

The filament needle used is not like a hypodermic needle you would see at a doctor’s office. A filament needle is very thin, solid, and flexible, where as a hypodermic needle is relatively thicker, hollow and has a blade tip. The filament pushes through the skin; it does not cut through the skin like a hypodermic needle would. The filament needles’ properties lend themselves to a much more comfortable entry. Once the needle enters the trigger point, the patient will feel the muscle “Grab”, twitch and then release. After the dry needling treatment, immediate improvement can be noted. However, soreness is usually accompanied, which can last on average 1-2 days, followed by relief of symptoms.

When can I get started?

Progressive Physical therapy is offering Trigger Point Dry Needling treatment in our offices where the Physical Therapist is a certified TDN. As of April 2009 the offices are limited, but expect other regions of South Carolina to have certified TDN Therapists in the near future.

Orthotics

Feet are the major weight-bearing part of the body and foot pain is a common ailment. The foot contains 26 bones, up to 2 sesamoid bones, and many small structures which support and balance the weight of the entire body. Walking puts up to 1.5 times one’s body weight on the foot, and humans walk an average of 1,000 miles per year.

Foot pain is not normal and should not be ignored. These problems can affect the functioning of other parts of the body, including the hips, knees, and back. These are foot-related problems that may be related to biomechanical issues.

Orthotics are prescribed to:

  • Provide support
  • Reduce the symptoms associated with many foot related problems
  • Provide better positioning
  • Relieve pressure on a certain area of the foot
  • Accommodate foot deformity
  • Improve the overall biomechanical function of the foot and lower extremity

At Progressive Physical Therapy’s Northeast Office, Deborah Pitts, DPT is fitting patients for custom foot orthoses as part of a treatment regimen. For those who are interested in Orthotics, an appointment should be made at this office. During your visit the therapist will make a cast of your foot, which is then sent to Sole Supports for approval and for your orthotics to be made. Turn around time is usually 10-14 days.

Patients are given some options, such as the color of the suede, color of the cushion, thickness, etc. Once the orthotics are in, the patient will be contacted to come back in to pick them up and for a fitting. At this time, the therapist can make any adjustments needed. Patients will also be given a shoe shopping guide for use with their orthotics along with general instructions.
